So, you want to participate?

All you need is a team. Team designations include:

Men's Teams (4 men)
Women's Teams (4 women) and
Co-Ed Teams (2 men and 2 women)

First, before recruiting your team for the 2011 CDPHP Workforce Team Challenge, please click on and read the following PDF documents:

3. Entry Procedures and Team Captain Information

Next, go the the forms at the bottom of this page and print out the Team Roster and Runner's Release Form, and if your CEO will be participating, print out the CEO Runner's Release Form. Everyone on your team must complete the Release Form and everyone on your team must be listed on the Team Roster.
